Evaluations of Senior Staff Members


About every four years faculty and staff at EHS have a formal opportunity to provide feedback on the effectiveness of their supervisors, recognizing the strengths and offering any suggestions/requests for change.

After choosing a set of logon ID# and password, each participant may access an online form that has twenty-two questions, with a rating scale provided. There are also four open-ended questions. Anonymity is provided, as responses from individual participants will not be available for review.

The combined results and randomly ordered comments are compiled into a summary report to be given to the person and his or her direct supervisor for analysis and discussion.
